It's presumptively linked to Gulf War-era burn pit exposure for veterans who served in the Southwest Asia theater.
The rating turns on how often sinusitis flares and how it is treated. Three or more incapacitating episodes a year needing four to six weeks of antibiotics, or more than six non-incapacitating episodes with headaches, pain, and discharge or crusting, supports 30%; one or two incapacitating episodes, or three to six non-incapacitating ones, supports 10%. 50% requires chronic osteomyelitis after radical surgery, or near-constant sinusitis after repeated surgeries. An incapacitating episode is one that requires bed rest and treatment by a physician.

How does the VA choose a rating level?
