"Hypercalcemia (indicated by at least one of the following: Total Ca greater than 12 mg/dL (3-3.5 mmol/L), Ionized Ca greater than 5.6 mg/dL (2-2.5 mmol/L), creatinine clearance less than 60 mL/min, bone mineral density T-score less than 2.5 SD (below mean) at any site or previous fragility fracture)." Note (2): where surgery is indicated, this evaluation continues until the day of surgery, when the 100 percent provisions take over. Note (3): where surgery is not indicated, it continues for six months after pharmacologic treatment begins, after which residuals are rated.
